The top tab includes spaces for the student’s name, ID number, school name and county, and entry and withdrawal information. The four sides of the folder include spaces for family information; grades, attendance, and comments for each grade for K-6 (10 years), 7-8 (3 years), and 9-12 (5 years); and a checklist for essential records such as birth certificate and health records. The comments sections can include special interests, observations, suggestions, and recommendations.
